en espanol Kindergarten Registration 2008-2009 School Year
Kindergarten registration for children 5 years of age on or before September 1, 2008 and residing within the boundaries of Cook County School District #84 will be held by appointment only at the school that they will attend on the following dates:
Tuesday, February 5, 2008 North Elementary School (847)678-7962 District 84 children residing north of Belmont and south of Addison.
Wednesday, February 6 and Thursday, February 7, 2008 Passow Elementary School (847)455-6781 1. District 84 children residing on Grand Avenue between Grand and the Milwaukee Road tracks and west of the Soo Line Tracks.
2. Those living in the 9600 block and 9700 block of Johanna, and the 2600 and 2700 blocks of Atlantic, Calwagner, Leona, and Reuter.
3. District 84 children residing east of the Soo Line tracks and south of Belmont.
Friday, February 8, 2008 Pietrini Elementary School (847)455-7960 District 84 children residing south of (but not on) Grand Avenue, and west of 25th with the exception of those living in the 9600 and 9700 blocks of Johanna, and the 2600 and 2700 blocks of Atlantic, Calwagner, Leona, and Reuter.
All testing will be done by appointment only.
Please call the school your child will attend to set up a time for testing.
All children are required to be present at registration.
In order for registration to be considered complete, you must provide a certified copy of the child's legal birth certificate. A certified copy of the legal birth certificate must be presented at the time of registration. Baptismal and hospital certificates are NOT acceptable. Birth certificates for children born in Chicago and Cook County suburbs may be obtained from the County Clerk's office, 1311 Maybrook, Room 109, Maywood, Illinois 60153. For information on fees and procedures, call (708)865-6010. Hours are Monday, Tuesday, Thursday, and Friday, 8:30am to 4:00pm. Wednesday hours are from 8:30am to 7:00pm.
By board policy, proof of residency upon initial enrollment is required. Pupils will not be permitted to start school until residency is established. At least one (1) of these proofs must be provided before initial registration will be accepted-two (2) are required after thirty (30) days.
The following documents may be used:
a. The most recent real estate tax bill for my residence, mortgage papers, or closing statement for the purchase of my home. b. A notarized copy of a lease showing the name, address and phone number of the property owner. c. Gas, electric, cable, or telephone bill (cell phone will be allowed as a second proof of residency). d. Public Aid card/documents. e. Home or apartment insurance certificate. f. Car insurance bill/car registration/driver's license (all 3 needed as one proof-state identification card is not acceptable.
Furth information may be required, and an investigation may be conducted by the school district to determine the residency of any student either before or after enrollment. The principal may refuse to enroll a child in cases where sufficient proof of residency is not provided.
Warning: Illinois law has made it a crime punishable by imprisonment and fine, to knowingly or willfully present any false information regarding the residency of a student for the purposes of enabling that student to attend on a tuition free basis or to knowingly enroll or attempt to enroll a student on a tuition free basis when the student is known to be a nonresident of the district. The school district will seek prosecution to the full extent of the law of any person who the district believes has committed any residency related crime. Additionally, a civil lawsuit may be initiated by the school district.
Physical and Dental Examination Illinois State Law requires that before entering school, children have a dental examination and a complete physical with immunizations brought up to date. Forms will be provided at the time of registration. Please bring your child's immunization record to registration for the nurse to review.
